Outreach explained
Residential or live-in care may not be what you feel you require at this point in time, so we have introduced our outreach service for all individuals living in the community. We have a team of dedicated outreach carers who will visit you on agreed days and times to support you with maintaining your independence.
The support our outreach team can offer are:
- Shopping
- Meal preparation
- Health and wellness visits to GP, hospital or clinic
- Trips out in the car
- Companionship
- Medication support prompt and encouragement.
Feedback shows introducing care and support into your daily life whilst it is still a choice can help prevent crises, such as:
- Falls and slips
- Hospital admission
- Loneliness
- Isolation
- And much more.
Our team will take your lead whilst supporting you to navigate your independent care choices.
